What you must do:

• Submit the waiver in person or via mail to PO Box 690, Newark, NJ 07101-0690.
  If sending via mail, please be sure to include a self-addressed stamped envelope.

What it will cost:

• $15.00 per estate, payable by cash, US Postal money order, or certified check.
